1. Schedule an appointment to complete a US DOT medical exam. Here
is a list of certified DOT medical examiners.
The DOT medical exam is required by most state
DMVs to obtain your Commercial Learner’s Permit and Class A CDL. Completing
this exam will satisfy both NLC’s admission and the DMV’s requirement.
2. Upon completion of the exam, make
sure the provider’s office gives you a copy of their completed Medical
Examination Report Form (4 pages) and a copy of the Medical Examiner’s
Certificate (1 page).
3. Take a picture or scan both documents and upload them below.

IMPORTANT: Please
ensure all fields, including signature and date fields, are completed by you
and the examiner.
Be sure to retain your DOT medical release paperwork for your records. It will be required when you obtain your CDL and future employers may request a copy.
